Forms & Intake

Ask before the visit. Show the reviews after it.

Pre-visit intake asks your questions right after an online booking, can stop the booking before payment when an answer is a warning sign, and shows the doctor the answers in the visit. Patient reviews come from completed visits in the patient portal, and nothing goes public until your clinic approves it.

Watch this update

Two things happen around every visit. Before it, the doctor needs to know why the patient is coming. After it, the next patient wants to know what others thought.

This update covers both, step by step. Pre-visit intake asks your questions on your website right after an online booking. Patient reviews let patients rate a completed visit, and you decide what goes public.

Part 1: Pre-visit intake

Where to find it

In the sidebar, under Marketing, open Pre-visit intake. The page has two tabs: Forms and Red flags.

Owners and managers can open it. So can anyone with the settings permission or the forms permission.

Step 1: Build the questions

Build the questionnaire in Info Templates › Forms with the normal form builder, and keep the form active. You can ask for short or long text, a number, a date, a time, a star rating, one choice, many choices, a dropdown, or a file or photo (up to 3 per question).

Step 2: Attach it to bookings

In Pre-visit intake, on the Forms tab, press Bind a form. Pick:

  • the form
  • Visit mode: Telemedicine or In person
  • Specialty, from the full specialty list
  • optionally one doctor or one service
  • Active

An empty field means "any". When more than one binding fits a booking, the most specific one wins: doctor, then service, then specialty, then your clinic default. The page explains this order too.

Step 3: Add red-flag rules

Open the Red flags tab. Press New rule, or start from one of four drafts under Start from a draft: chest pain, stroke signs, thoughts of self-harm, and bleeding during pregnancy. Each draft comes with guidance text in English and Arabic. Review it and change it to fit your clinic.

A rule says:

  • which question to watch
  • When the answer: is exactly, is one of, contains, is not, was answered at all, is at least, or is at most
  • Severity: Emergency or Urgent
  • Action: Block booking stops the booking before payment. Warn only lets the booking go on
  • the guidance title and text the patient sees, in English and Arabic
  • where the rule came from

Step 4: Sign off each rule

A new rule shows Unsigned. A manager reviews it and presses Sign off. The rule then shows Signed off with the date, and your clinic knows who approved it and when.

The page warns you in red while active rules are still unsigned. An unsigned rule still works. If you edit a rule, its sign-off is cleared, and the form tells you before you save.

What the patient sees

The patient books online on your website. The booking is saved first. Then, before any deposit or payment, they see your questions, with a note that the booking is already saved.

  • They press Send answers, or Skip for now and finish later.
  • A telemedicine booking also shows your consent text with an agree checkbox.
  • If a Block booking rule matches, payment is replaced by your emergency guidance. With no title of your own, it says Please seek care now. The booking stays saved.

Finishing later: the booking status page shows a short card with Answer now. Answers they already gave come back filled in.

What the doctor sees

In the doctor room, the visit details show a Pre-visit intake card, for in-person and video visits, on a computer or a phone:

  1. Red flags first: red for Emergency, orange for Urgent.
  2. A note if the booking was stopped with emergency guidance before payment.
  3. Then the answers. The rest fold under "more answers".

Flagged answers are marked. Photos show as small previews, and files as links. Questions and choices show in the doctor's language.

Part 2: Patient reviews

Where to find it

In the sidebar, under Marketing, open Patient Reviews. At the top it says: nothing appears on the public site until it is approved here.

Anyone with the settings permission or the marketing permission can use it. The tabs are Pending, Published, Replies, Rejected, and Withdrawn.

How a patient leaves a review

A patient with a portal account can review any completed visit with a doctor, in the clinic or by video. One review per visit.

  1. In the patient portal they see How was your visit? and press Review this visit. Signed in on the doctor's page, they can also press Write a review.
  2. They pick 1 to 5 stars, and can add a headline and a few words.
  3. Show my name as lets them pick a name. Left empty, only their initials show. Their full name never shows.
  4. A short reminder asks them not to write medical details.
  5. They press Submit review. The review shows Waiting for approval until you publish it, then Published.

They can press Remove my review at any time, and it leaves your site right away.

How your team approves reviews

  1. Open Pending. Each row shows the doctor, the stars, the text, and the date.
  2. Open a review. You can add an internal note. The patient never sees it.
  3. Press Publish or Reject. A published review can come down later with Take down.
  4. Many at once: tick the rows, then Publish selected or Reject selected.
  5. To answer in public, write a Clinic reply, press Save reply, then Publish reply. It shows under the review as a reply from the clinic. It carries the doctor's name only when the doctor wrote it. Every reply is in the Replies tab.

A rejected review stays private.

Where reviews show on your website

  • Doctor directory: every doctor card shows the stars, the average, and how many reviews, or No reviews yet. Highest rated does not let one 5-star review jump over a doctor with many good reviews.
  • The doctor's page: a reviews wall with the average, a 5-to-1 breakdown, each review tagged Verified visit, your reply, and Show more reviews.
  • Google: once a doctor has 3 published reviews, the doctor's page tells search engines the star rating. Google decides whether to show stars in results.

One setup step: the default doctor page, Staff profile, has no reviews wall. To show it, apply Staff profile — with reviews in the website builder. The three telemedicine site kits already use it. In the builder, the section is called Patient reviews. You can set its heading, how many reviews show at a time, whether the breakdown shows, and whether patients can write a review from the page.

Good to know

  • Intake works with online bookings on your website. Bookings made by reception do not get the questions.
  • Your team does not get an alert for a red flag or a new review. The doctor sees the intake when they open the visit, and reviews wait in Pending.
  • Patients review from the patient portal. There is no review request by WhatsApp yet.
